We would like to assure that all reported violations will be handled with strict confidentiality when it comes to the whistleblower information and identity, and the whistleblower is protected against any form of retaliations.

Kindly ensure to cover the following information prior submitting your concern:

What is the type of violation that occurred.
Please provide more details about the violation (or the event that occurred).
Where did the violation take place (location)?
Do you have any evidence to support the report? If yes, please attach in the email.
Do you have any other details or information which would assist us in the investigation? If yes, please specify.
What was the date the violation occurred?
When did you notice the occurrence (date)?
Is it on-going and/or frequent?
Are there any other parties involved other than the suspect stated above? If yes, please specify.
Are there any witnesses to the event? 
Have you reported this violation to anyone else? If yes, please specify.

Kindly submit the information in good faith and treat the report as strictly confidential while an investigation is in progress. And ensure not disclose the content of this report or findings of the investigation to any other employees or third parties unless authorised to do so or as provided by applicable law.
